Informed Consent for Telehealth Services

1. Purpose of This Consent

This document provides important information about telehealth services coordinated through TuFlorecer. By proceeding with our services, you acknowledge that you have read, understood, and agree to the terms described below.

Telehealth involves the delivery of healthcare services using electronic communications, including the review of health questionnaires, asynchronous provider evaluations, secure messaging, and potentially video or phone consultations. These services are provided by independent licensed healthcare providers — not by TuFlorecer.

2. Nature of Telehealth Services

The telehealth services available through TuFlorecer may include:

  • Asynchronous review of your health intake questionnaire by a licensed physician or clinician.
  • Clinical evaluation to determine eligibility for prescription weight management medications, including compounded semaglutide with vitamin B12 (cyanocobalamin) combination formulations.
  • Electronic prescribing (e-prescribing) of medications to licensed compounding pharmacies for fulfillment and delivery.
  • Ongoing monitoring and follow-up consultations as determined clinically appropriate by your provider.

3. Potential Benefits of Telehealth

  • Convenient access to licensed healthcare providers without the need for in-person office visits.
  • Ability to receive medical evaluations and treatment from the comfort of your home.
  • Reduced wait times compared to traditional in-office appointments.
  • Access to care at flexible times and in your preferred language (Spanish or English).

4. Risks and Limitations of Telehealth

  • Telehealth has limitations compared to in-person care. The provider cannot perform a physical examination, which may limit their ability to diagnose certain conditions or identify issues that would be apparent in person.
  • The information transmitted during a telehealth consultation may not be sufficient for the provider to make a complete medical assessment. The provider may determine that telehealth is not appropriate for your situation and recommend in-person care.
  • Technology failures, including internet disruptions or platform outages, could delay your care or result in loss of transmitted information.
  • While we implement robust security measures, electronic communications carry inherent risks to data privacy. Security protocols could fail, potentially resulting in unauthorized access to your health information.
  • Treatment options — particularly prescription medications — may be limited compared to in-person consultations due to state regulations, clinical guidelines, or the nature of telehealth evaluations.

5. Compounded Medications

If your provider determines that medication is clinically appropriate, you may be prescribed a compounded formulation containing semaglutide in combination with vitamin B12 (cyanocobalamin) or other ingredients. These compounded medications are prepared by licensed 503A/503B compounding pharmacies in FDA-regulated facilities.

You understand that compounded medications are not FDA-approved and have not been independently evaluated by the FDA for safety, efficacy, or quality in their compounded form. The decision to prescribe compounded medication is based on the provider's independent clinical judgment after reviewing your medical history.

Common side effects of semaglutide-based medications include nausea, diarrhea, vomiting, constipation, and abdominal pain, particularly during the initial weeks of treatment. Serious but rare side effects may include pancreatitis, gallbladder problems, kidney issues, and allergic reactions. GLP-1 medications carry a boxed warning about the risk of thyroid C-cell tumors. Medications may not be shipped in child-resistant packaging. You are responsible for storing all medications in a safe location out of the reach of children and pets.

6. Your Responsibilities as a Patient

  • You must provide truthful, accurate, and complete information about your health history, current medications, allergies, and medical conditions. Providing false or misleading health information may result in inappropriate treatment and could cause serious harm, including death.
  • You are responsible for following all treatment instructions provided by your healthcare provider, including medication dosage, administration technique, and follow-up care.
  • You must inform your provider of any changes in your health status, new medications, or adverse reactions promptly.
  • You should not use medications prescribed through this Platform while pregnant or breastfeeding unless specifically cleared by your provider.
  • You agree to seek immediate emergency medical attention if you experience severe side effects, signs of an allergic reaction (difficulty breathing, swelling of face/throat), or symptoms of pancreatitis (severe abdominal pain).

7. Acknowledgments

  • I understand that TuFlorecer is a technology platform and not a healthcare provider. Medical services are delivered by independent licensed healthcare providers.
  • I understand that telehealth services are not a replacement for in-person care and do not replace my relationship with my primary care physician.
  • I understand that no specific results, cure, or outcome are guaranteed. My condition may not improve, or may worsen, despite treatment.
  • I understand that the provider may determine that I am not a suitable candidate for treatment and may decline to prescribe medication.
  • I authorize the collection, use, and sharing of my health information for the purposes of treatment, payment, and healthcare operations as described in the Privacy Policy.
  • I understand that my telehealth records will only be shared with other healthcare providers with my explicit consent and request.
  • I understand that technical failures may result in delays or loss of information, and I agree to hold TuFlorecer harmless for any delay in care caused by technology issues beyond its reasonable control.

8. Right to Withdraw Consent

You may withdraw your consent to telehealth services at any time by contacting us at info@tuflorecer.com or by canceling your subscription through your account dashboard. Withdrawal of consent will not affect any care or services you have already received.

Your consent is automatically renewed upon each new telehealth consultation or service interaction unless you withdraw it. Withdrawal of consent may limit your ability to continue receiving services through the Platform.

9. Medication Shipping and Delivery

Medications prescribed through our affiliated providers are dispensed by licensed compounding pharmacies and shipped via cold-chain delivery services to your provided address. Medications are considered dispensed when signed out for shipping by the pharmacy — not upon delivery to your address.

TuFlorecer is not responsible for shipping delays, delivery errors, or damage caused by third-party carriers. If you receive a damaged or incorrect shipment, contact our support team to coordinate a resolution with the pharmacy.

10. Prescription Transfer

Your prescription may be transferred between licensed pharmacies in our network as needed to ensure timely fulfillment, quality, and availability. You will be notified if your prescription is transferred to a different pharmacy.

You have the right to request that your prescription not be transferred, or to request that your prescription be sent to a pharmacy of your choice. Contact our support team at info@tuflorecer.com for assistance.
